Unable to add new sources

I am having trouble using v1.11
Every time I try and add a new source to an event, when I select the tick on the source reference i get an error which says "key violation"
Is there any way to solve this?

Please use "File | Database | Pack" to rebuild the table indices.

If this does not solve the problem, please exit K&KPro and use "Start | Programs | Kith and Kin Pro | Borland database fix utility" to rebuild the Sources, Documents and Repositories tables. You may then need to run "File | Database | Pack" again.
